Mind in Bradford is proud to be providing funding for community grants in a bid to tackle health inequalities.
The aim of this funding is to deliver evidence-based physical health interventions for people with a severe mental illness, to improve their physical health across three Primary Care Networks (PCNs): PCN 6, PCN 9/ BD4+ and Modality.
Mind in Bradford is providing this funding under the CCG’s system brand Healthy Minds and as part of the community mental health transformation.

If you are part of or know of a voluntary and community sector organisation working within the Bradford district that has a great idea to improve the physical health of those living with a severe mental illness, we encourage you to apply.
The application deadline is 5pm Thursday 27 January.
FREE SMI Training for all applicants on Zoom
If you would like to sign up for our free SMI and physical health checks training, please sign-up on Eventbrite.
The training is free to attend and is open to staff working in VCS and other organisations in Bradford, Airedale, Wharfedale and Craven, supporting clients with their mental health.
