Bradford District and Craven Mind is one of the leading providers of Community Outreach Recovery and Engagement (CORE). This service is part of the Bradford District & Craven Community Mental Health Transformation Programme.
We provide comprehensive support for people with Severe Mental Illness (SMI). Our community mental health workers assess people’s needs and help them access community services and resources that support mental wellbeing and address related issues. If you are registered at the following GP practices, please speak to your GP or ARRS worker to make a referral to our CORE service.
