Personal Health Budget Practitioner

If so, we look forward to hearing from you! The aim of this service is to provide support for inpatients and utilising their PHB to facilitate personalised care back into the community, this can be done by providing goods or a service to ease their transition of care from inpatient to community services.

• Have experience of working with adults in a health, social care or any other relevant setting

• Willingness to travel across the District (Full UK driving licence & have access to a car)

Important information 

Salary: £12 – £13.14 per hour

Contract: Temporary for 12 months with the potential for extension                                                       dependent on funding.

Hours: 20 hours per week

Probation: 6 months 

Closing Date: Sunday 19^th May 2024

Interview Date: Wednesday 29^th & Thursday 30^th May 2024

Applicants to be able to work evenings and support the on-call rota as and when required
