The information is collected for research purposes and will not affect your involvement with Mind in Bradford in any way. Summaries of demographic information may be shared with funders, but it will not include personal details and cannot be linked back to an individual. This code of conduct has been developed based on feedback from clients. Its purpose is to ensure all people and groups feel welcome, safe and valued at Mind in Bradford.

DECLARATION AND SIGNATURE

All of your personal information is processed securely in line with data protection regulations. Please read, sign and date the declaration below:

Hearing Voices support is offered in the form of a weekly peer support group or as 1-1 support via the Maastricht Interview Technique. Clients must have stable and secure accommodation, have no overdoses, suicide attempts or active plans for suicide in the last 6 months, as we need to ensure a level of stability before we start the 1-1 support. You can still be added to the waiting list and reviewed at that 6-month point.
