Know Your Mind is provided by Bradford District and Craven Mind and is part of the Youth in Mind partnership.

How can Know Your Mind support me?

Know Your Mind supports children and young people aged 5-19 or up to 25 with additional needs with 8-weeks of one-to-one support.

Who can access this service?

To access support you must live, go to school in, or attend a GP surgery in postcodes starting with BD1 up to BD9, or Keighley central.

What happens after completing your referral form?

Once we’ve received your referral form a Community Wellbeing Worker from Bradford District and Craven Mind will be in touch to schedule a ‘get to know you’ session. This is an informal chat so we can find out how we can support you best.
